New Patient Forms at Gardena Dental Care

Welcome to Gardena Dental Care! We offer our new patient forms online for your convenience. Simply print them, fill them out, and bring them to the office with you to save time on the day of your appointment.

If you don’t have access to a printer or would rather fill them out at the office, don’t worry, you can do that too. Just tell the front desk staff that you need the forms when you check-in and they’ll be happy to get them for you.

Our new patient forms are simple and easy. We need your patient information, including your insurance information, so that we can get to know you. We will also verify all of your insurance information so we can give you correct information regarding your out-of-pocket costs. This will streamline billing and make it easier to give you an accurate treatment plan.

We’re also required to collect basic information such as your contact information and emergency contact person.

The second form we need includes your medical history. You might be wondering why you need to give us your medical history for a dental appointment. It’s because we want to give you the best possible dental care and your medical history affects your teeth, just as your dental health affects your overall health.

Some medical conditions impact dental care (and vice versa) and some medications will interfere with medications used in dentistry. We also need to know if you have any allergies (such as to latex or novocaine) so that we can take the proper precautions. You’re a whole, complete person and we want to make sure we treat you as such.

Don’t worry about the privacy of your information -all your records are stored confidentially in accordance with the federal HIPA (Health Insurance Portability and Accountability Act) privacy law.
